Note: This checklist was prepared as a guide for people who are considering the formation of a nonprofit corporation. It should not be considered as a complete list of the steps that will be taken. Depending on the state, then type of program, facility and sources of funding, additional registration, reporting or licensing may be required.
[ ] Determine
purpose and structure.
[ ] Form founding board of directors.
[ ] File articles of incorporation.
[ ] Draft bylaws.
[ ] Determine program plan.
[ ] Develop a budget.
[ ] Draft a fund-raising plan.
[ ] Hold formal organizational meeting.
[ ] Establish a record-keeping system.
[ ] Request, then file application 1023 to the Internal Revenue Service.
[ ] File charitable trust registration with the state.
[ ] Apply for solicitation license from local municipality.
[ ] Establish a bookkeeping/accounting system.
[ ] Develop and submit fund-raising proposals.
[ ] File employer registration with federal and state government
agencies for income tax withholding.
[ ] Decide about Social Security contributions.
[ ] File for state income tax exemption.
[ ] Apply for state sales tax exemption.
[ ] Register with state unemployment insurance program.
[ ] Apply for property tax exemption.
[ ] Obtain liability insurance.
[ ] Apply for nonprofit bulk mail permit from post office.
[ ] Develop personnel policies.
[ ] Begin program activities.
